Targeted metabolomics is a qualitative and quantitative analysis method for several target compounds of some or all metabolites involved in a metabolic pathway, using standards as references. In comparison with untargeted metabolomics, targeted metabolomics has the characteristics of high specificity, high sensitivity and accurate quantification. Therefore, targeted metabolomics is suitable for further validation of key metabolic molecular-markers discovered by untargeted metabolomics. Furthermore, targeted metabolomics can directly target a specific class of metabolites for quantitative analysis and combine with other histological data to elucidate the relevant molecular biological mechanisms.

Ketone bodies are products of lipolysis metabolism. It can be used as a substitute substrate for glucose as an energy source when carbohydrate intake is low and circulating free fatty acids (FFA) are excessive.

Elevated ketone bodies occur when insulin is insufficient to utilize glucose as an energy source during prolonged fasting or a carbohydrate-restricted diet. The major ketone bodies are ß-hydroxybutyric acid (ß-HB), acetoacetic acid (AcAc), and acetone. Under normal physiologic conditions, total plasma ketone concentrations fluctuate between 100 and 600 µM, and can rise to approximately 1 mM after prolonged exercise or 24 hours of fasting.

Diabetic Ketosis is the most common acute complication of diabetes and is diagnosed by ketone body concentrations > 1-1.5 mM. The urine ketone body test mainly responds to acetoacetic acid and is less responsive to acetone. Blood ketone bodies mainly respond to ß-hydroxybutyric acid in the blood. Nevertheless, ketone bodies in the exhaled breath primarily respond to acetone’s level.

The measurement of ketone body concentrations is important for the diagnosis of ketoacidosis. Ketone bodies can also be used as biomarkers to observe changes in metabolic pathways in individual patients during exercise, diet or lifestyle management programs, and medication.

Mass spectrometry and nuclear magnetic resonance (NMR) are common technical platforms for metabolome analysis. To better develop methods for the clinical quantification of the three major ketone bodies by NMR instruments, researchers at Laboratory Corporation of America Holdings (LabCorp) compared NMR with liquid chromatography coupled with tandem mass spectrometry (LC/MS/MS) or gas chromatography/mass spectrometry (GC/MS), to quantify ketone bodies. Creative Proteomics contributed to this study by measuring the levels of ß-HB and AcAc by LC/MS/MS and acetone by GC/MS. The results showed that NMR was not a good analysis of AcAc levels, and ß -Hb detection may not be suitable for real-time detection. Thus, NMR detection methods need to be further explored, and the current method based on mass spectrometry is indispensable.
